Client Forms & Using SimplePractice

The Counseling Center uses SimplePractice for the majority of its medical records processes. We will send you a set of forms to complete electronically prior to your first appointment through the Client Portal on SimplePractice. Please complete those forms prior to seeing your therapist.

If for some reason you cannot complete forms electronically, due to either a lack of access to a computer or difficulties using electronic equipment or processes, please let us know and we will supply you with paper copies of the forms during your first appointment.

Completed forms, plus all your appointment records, will be maintained on SimplePractice, a HIPAA compliant platform used by clinicians worldwide.

The use of SimplePractice allows us to respond to requests for records with greater ease, as well as automate our processes for greater efficiency.

We hope you will find it simplifies the intake process as you begin therapy.

These forms ask basic information about your:

  • Telephone, email and address information
  • Emergency contact’s telephone, email and address information
  • Medical history
  • Current prescriptions/medications
  • Prior experience in psychotherapy
  • Presenting concerns
  • History of drug and alcohol use
  • History of trauma, if any
  • Initial goals for therapy
